I AUTHORISE TUDOR LODGE TO ARRANGE VETERINARY ATTENTION SHOULD IT BE NECESSARY. I UNDERSTAND THAT FEES PAID/DUE ARE NON-REFUNDABLE/DUE IN FULL IF THE ANIMAL IS COLLECTED PRIOR TO PRE-ARRANGED COLLECTION DATE.
